Telehealth Nurse (onsite)

We seek an energetic full-time Client Specialist RN to join our Valhalla Vitality Team to work in a cutting-edge alternative medicine treatment environment. Responsibilities

• Client intake and initial assessment of patient health needs and goals
• Evaluations include a determination of health status, strengths, and care needs
• Review consumer medical documentation and or health referral forms as relevant to the case
• Enter evaluation data into electronic evaluation form and transmit, as required
• Efficiently guide the client toward the appropriate therapies offered by Valhalla Vitality
• Discuss the medication profile with the client including medication uses & side effects
• Document any concerns, conflicting information, or other issues that surface during the evaluation process
• Emphasize continuity of care, thus reducing or eliminating fragmentation, duplication, and gaps in treatment plan
• Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery
• Discuss health care options, supports needed, service vendor options, and waiver options
• Collect quality review data and any required documentation to support outcome measurements and record case notes

Qualifications:

• Bachelor’s of Science Degree from Accredited Registered Nursing Program
• Active Registered Nurse license in New York State
• At least one year of related work experience as an RN
• CPR and all relevant specialty certifications required
• Prior experience using the UAS-NY Community Assessment tool, OASIS, or MDS
• IV therapy experience and training
• Ability to work with clinical technology to fulfill process and workflow needs
• Ability to multitask, prioritize in a fast-paced environment, and meet deadlines with minimal supervision
• Strong communication and client care skills
• Active Driver’s License
• Ability to work independently without supervision but keeping an open line of communication with management
• Ability to work under pressure
• Exceptional interpersonal skills

Job Type: Full-time

Salary: $40.00 – $45.00 per hour

Expected hours: 40 per week
